Has use changed from last inspection? Are there any areas of exposed ash? Design concerns (if applicable) Current use is an open field. Fire ants have brought ash to the surface in several places, otherwise we did not observe any exposed ash. The structural fill area was overgrown, and we were only able to inspect the south slope and part of the top. It should be re -inspected in the winter. At the bottom of the structural fill's south side slope there was a wetlands area and creek. We observed a brownish red liquid flowing into the wetlands area, possibly leachate, see photos below. A tracker trailer, boat and some large machinery was being stored on top of the fill. What is the proximity of the structural fill to receptors? Private Dwellings — no, see Figure 1, Proximity Map. Community Centers — no. Surface Water Features 50' (Lakes, Ponds, Wetlands, Streams) — yes, see Figure 2, Wetlands Map. Are there any known utilities in close proximity to, within and/or beneath the fill area? — none known Past Noted Violations: (What were they and were the violations resolved?) On August 18, 2009, the Solid Waste Section issued an NOV for impounded water over waste, eroded side slopes, coal ash deposited in wetlands, side slopes were too steep, and failure to record. This NOV resulted in a compliance order dated April 13, 2010. On November 10, 2010, Mary Whaley re -inspected the site and noted the following in her inspection report: "Violations from August 18, 2009 NOV, appear to be resolved. A letter from Ervin Lane, NCDENR, SWS, Hydrogeologist, concerning soil samples analyses, is needed prior to the official resolution of this compliance order." On June 1, 2009, the Division of Water Resources issued a Notice of Violation for stockpiling coal ash in a wetland and water quality issues in the wetlands and nearby stream. DWR also issued an NOV on October 21, 2016 for water quality issues in the nearby wetlands exceeded wetland standards. These NOVs has not been resolved, as far as the solid waste section is aware. m The structural fill project on Highway 46 is located near Garysburg, NC. According to the Northampton County Tax Assessor it is identified as Parcel # 4918-28-8013. The property is owned by Henry B. Long. Approximately 480,612 cubic yards of coal combustion products were utilized in the construction of the structural fill on 21.7 acres. The structural fill operation was performed by Plumbline Corp, P.A. The coal combustion products were generated by the Roanoke Valley Energy Facility in Weldon, NC. The stated purpose of the structural fill in the Notification received by the Solid Waste Section on April 22, 2004, was "to construct an Industrial/Commercial site for a private developer. The end -use of the site will be for office buildings..." As of the date of the inspection, no buildings have been constructed on the property. The site was recorded by the property owner, Henry Bourne Long, with the Northampton County Register of Deeds on April 17, 2009.
