Please respond and/or submit requested document in a timely manner. The CQA Plan1. (1. Introduction, A. Purpose) The Contract Specifications is mentionedthroughout the CQA Plan. Please provide the Contract Specifications approvedby the Engineer. 2. (Introduction, C. Parties) Please specify who is the ENGINEER responsible forthe preparing of the Contract Drawings, Technical Specifications, and the CQAPlan. Because Joyce Engineering, now owned by LaBella Associates, preparedthe above-mentioned documents for this slope repair project; it is likelyassuming LaBella Associates is the ENGINEER for this project. Additionally,the ENGINEER must register as a professional engineer in the State of NorthCarolina. Please add the requirement to this subsection. 3. (2. Earth Materials, D. Documentation/Certification, General) The meetingminutes including the sign-in sheet stated Section 1 - Introduction, D.Communications and Meetings should be a portion of the CQA report for thisproject. Please add the requirement to this subsection. 4. (2. Earth Materials, D. Documentation/Certification, Certification) In the CQACertification Report, the CQA Consultant should state any deviation to theapproved CQA Plan, Contract Specifications, or Drawings and/or non-conformance matters and resolutions. Slid Analysis Information and Drawing No. CP-015. The final slope angle of 2 to 1 shows on Drawing No. CP-01; please provide thedata of slope configurations or geometry in the Slid Analyses Information toconfirm the consistence of the final slope angle of 2 (horizontal) to 1 (vertical). Stormwater system design and erosion and sediment control6. The stormwater design and erosion & sediment control for the sloperepair/restoration project will modify the existing erosion and control permit forthe industrial landfill facility. Please submit a approval of the modification tothe erosion and sediment control permit issued by the NC Land Quality Sectionor Forsyth County. Please contact me if you have any questions or require further clarifications on the above- mentioned comments. The submittal isincomplete and not acceptable one, because it is not signed and sealed by a professional engineer registered in the State of North Carolina as mentioned in the email message date02/19/2019 (see attachment). This engineer is fully responsible for this slope design, calculations, CQA plan pertaining to the slope restoration at OmniSource SoutheastIndustrial Landfill in accordance with North Carolina General Statute (N.C.G.S.) 89C. Upon receiving the completed document, the Solid Waste Section will only review to see if the submittal satisfies the minimum standard of practice of the slope restoration that integratesinto the inactive industrial landfill, Permit # 34-20. We plan to finalize the plan this week, after meeting with the contractor, our insurancecarrier, and the insurance carrier’s engineering firm. If there are any changes, I will forward those to you when available.
