The submittal is incomplete and not acceptable one, because it is not signed and sealed by a professional engineer registered in the State of North Carolina as mentioned in the email message date 02/19/2019 (see attachment). This engineer is fully responsible for this slope design, calculations, CQA plan pertaining to the slope restoration at OmniSource Southeast Industrial Landfill in accordance with North Carolina General Statute (N.C.G.S.) 89C. Upon receiving the completed document, the Solid Waste Section will only review to see if the submittal satisfies the minimum standard of practice of the slope restoration that integratesinto the inactive industrial landfill, Permit # 34-20. We plan to finalize the plan this week, after meeting with the contractor, our insurance carrier, and the insurance carrier’s engineering firm. The determination of limits of failure slope surface. “2018 Slope Failure Survey” is marked on Slope Profile in Drawing No. CP-01. Is the failure slope surface determined/confirmed by a subsurface investigation/soil testing report. The report should be a portion of the Plan which must be signed by an engineer or geologist registered in the State of North Carolina. 2. Slope design. According to the Drawing No. CP-01, the new slope will be constructed with a 2 (horizontal) to 1 (vertical) angle- approximately 26.5 degrees. (i) There is no word about what kind material to be used for slope restoration – soil with vegetative cover, 100% hard amour including concrete/broken rock/riprap/gabion box, or soil with a hard amour system (ii) Is there a slope stability analysis to support this design? A copy of the design should be a portion of the Plan. (iii) The engineering parameters of the material for the slope re-construction (including density, strength, type, moisture, etc.) that used in the slope stability analysis must be confirmed by the QA/QC testing program through the established Construction Assurance Plan and technical specifications (see Item 3). The slope design must be approved and signed by a professional engineer registered in the State of North Carolina. 3. The Construction Assurance (CQA) Plan and technical specifications for slope restoration project. Plans should describe the responsibility of each parties involved into this project and construction and material specifications, QA/QC testing requirements (testing methods and testing frequencies), and the final as- built drawings and construction completion report. The inputs and comments from the contractor – Baker Construction Services and his subcontractor and the engineering consultant contracted by your insurance company should be incorporated into the CQA Plan. 4. If you agree, the primary reason causing this slope damage is the failure of the existing stormwater drainage system which had structural and hydraulic flaws - the old drainage system could not carry/convey the storm water flow produced by the hurricane – flow capacity problem; the drainage system is eroded by the flow with velocity/erosive force exceeding the ones designed for. The Plan fails to consider the storm water drainage system on the top side of the slope. The new stormwater system must be designed by a professional engineer registered in the State of North Carolina. Additionally, the new 2 to 1 slope has not benching or other lateral drain devices/measures to prevent erosion from surface runoff (you and I told about it on 10/05/2018 sit visit). The stream bank protection at the downslope area is not available, either. 5. The final construction completion report and as-built drawings should be certified by a professional engineer registered in the State of North Carolina. 2 The above-mentioned requirements are the minimum and based on my 30-yrs experiences in the civil/environment engineering and construction business and NOT required by any NC Solid Waste Management Rule. Whether or not to accept my suggestions, I leave this entirely up to your discretion of course. Send all suspicious email as an attachment to report.spam@nc.gov Attached is a copy of Joyce’s Slope Grading and Repair Plan for the northern slope of the landfill at our Kernersville facility, which was damaged during Hurricane Florence. A copy of this plan has been submitted to our proposed contractor, Baker Construction Services. Although Baker had some technical questions, it was their opinion that the repair work could be completed as designed. We have also submitted a copy of the plan to our insurance company, who has contracted with another engineering firm to review. Once we get this review back, I should be able to finalize the revised project schedule – at this time I would conservatively estimate repairs to be completed by the end of June.
