OBSERVED VIOLATIONS: None. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS On March 12, 2019, Chuck Kirchner, Kim Sue and Ming Chao met with Billy Helms to conduct a partial permit inspection of IFS Services, LLC Medical Waste Treatment Facility on Alton Place in High Point, Guilford County. Facility inspection is per request by Mr. Helms in order to close the site permit and release the Financial Assurance. 1.The facility was a medical waste treatment and processing operation consisting of an autoclave.2.The facility also operated as a medical waste transfer station.3.The facility received regulated and non-regulated medical waste.4.A healthcare uniform laundering service is going to continue to be operated within the same building by IFS Services, LLC. The operations did not share the same space. The medical waste treatment operation was fully enclosed within its own sector of the building.5.Mr. Helms first contacted the Solid Waste Section on January 8, 2019 to discuss plans to cease operations and close the permit.6.The facility ceased operations on February 4, 2019 and the last load was transported to the landfill on February 6, 2019. 7.The autoclave, bins and refrigerated truck have been removed from the facility at the time of the inspection.8.A dust mask was observed in the french drain. Facility staff stated that this would be removed and disposed of properly. 9.The area the autoclave was in, to include the drains, is a concrete cut out section of floor that is ~2’ below the normal floor grade. Facility staff stated that this area was going to be backfilled with concrete and this section of floor brought up even with the rest of the facility for future use. 10.Curtis Bay Energy, LP from their Woodstock, Georgia facility are coming soon to take ownership of the two box trucks, roll-away dumpster and trash compactor per facility staff.11.The trash compactor, roll away dumpster, facility and box trucks were inspected and found to be free of any medical waste. The laundry facility has moved some crates, pallets, linen shrink wrap machine and other associated items from the laundry service into this area for extra storage.12.There was water noted in the roll-away dumpster. It was evident during the inspection that the dumpster interior had been thoroughly cleaned out. Facility staff also pointed out that the dumpster was not properly seated the last time it was dropped off at the facility which most likely led to rain water getting into the dumpster.13.The autoclave has been disconnected from the sanitary sewer system. This sanitary sewer system runs into a tank outside the building. This tank was installed to let the water from the laundry service cool down prior to going into the sanitary sewer. Facility staff removed the manhole cover and the contents were visually inspected during this inspection. Nothing of concern was noted inside the tank. The tank and the facilities discharge to the public sewer system are regulated under an Industrial User Permit from the City of High Point.14.The facility staff stated it maintains records of the amount of medical waste received by package, generator information, date received, date treated, and disposal facility. Records of medical waste transferred are also maintained. It was recommended that records be maintained at the facility for at least three years per permit requirements. 15.The facility annual report (FAR) was received by the SWS, dated July 8, 2018. Facility throughput for July 2017 through June 2018 is 124.32 tons.16.The facility signage is still in place. 17.The PTO is recommended for closure at this time from the Field Operations Branch. The current PTO expiration date is July 2, 2022. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
