PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 – This facility has required signage at the entrance and directional signs inside of the landfill footprint. 02 – Roads are constructed of all-weather materials and are being maintained well. Facility has locking gate and fencing for security. 03 – Edge of waste markers present. 04 – Asbestos area is marked. 05 – Monitoring wells are locked and tagged. 06 – Facility has adequate soil cover pile on-site for use. 07 – Incoming cover load tickets reviewed. 08 – Cover records were reviewed and are documented weekly at a minimum. 09 – Working face of this facility is kept small with cover material on working face. 10 – Incoming load of C&D waste was being screened on the working face during this inspection when arrived. 11 – No non-conforming waste observed on working face. 12 – Tires had been pulled out of several loads prior to the inspection and were in a pile ready to be taken to the scrap tire container at the on-site convenience center. 13 – There was a small amount of windblown trash due to high winds. Be sure to pick up windblown trash by the end of the working day. 14 – This facility has 5 certified Landfill Operations Specialist: Paige R. Aycock, expires 10/19/2019, Kenny E. Matthews, expires 10/19/2019, Melinda R. Wall, expires 10/19/2019, Jonathon Mark Denton, expires 11/04/2019 and Robert B. Patterson, expires 04/11/2020. Also, the facility has 1 Certified Technical Associate of Landfill Operations, George Andrew Davis, expires 05/06/2020. 15 – The Yard Waste area is managed well. Windrows are uniform and have numerical signs. Temperatures are being recorded and were reviewed. 16 – Some incoming loads of yard waste needs to be screened better to ensure all non-conforming waste (plastic bags, cans, cups and bottles) is removed before the yard waste makes it to the grinder. Several new loads were looked at with this issue. 17 – Land Clearing and Inert Debris area is well maintained and is being covered monthly at a minimum. 18 – Overall management of this facility is good. 19 – Digital pictures taken during this facility inspection.
