Issued April 12, 2018. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 – Permit to Operate was issued on April 12, 2018. This is a Life of Site Permit. 02 – Facility is permitted to receive 20,000 Tons per year. 03 – Permitted to receive waste from Martin County. 04 – Facility has 4 Certified Landfill Operations Specialist: Johnny Ray Rascoe, expires 12/06/2021, Justin K. Harrison, expires 09/27/2019, Ronald G. Brownfield, expires10/19/2019 and Kimley L. Brown, expires 09/27/2019. 05 – Last Groundwater monitoring sampling was done 11/28/2018. 06 – Scrap metal and white goods area looked good. All white goods with the CFC’s removed has an X painted on and all white goods are stored upright to prevent the CFC’s from leaking out. 07 – Scrap tires are laced into a closed top trailer. 08 – Pesticide container storage building is being managed well. 09 – Facility has signage with required information. Need to put the Un-acceptable waste sign back up on the gate that has fallen off. 10 – Roads are constructed of All-Weather materials. 11 – Cover records (logs) were reviewed but some (from September to current) were missing. Need to find the missing reports and put in cover log file folder. Please keep cover logs in office at least on a weekly or monthly basis so they don’t get misplaced. 12 – Screening records reviewed and has required information. Kept in scale house. 13 – In coming load tickets were reviewed and are kept in scale house. 14 – Landfill gas was last sampled on 11/27/2018. 15 – Cover material received records are kept in office and were reviewed. 16 – Working face needs more cover material. May need to keep the working face smaller and keep stockpiled cover material on working face to assist with the weather this year. 17 – Continue efforts to stabilize side slopes. 18 – Due to wet conditions from this year’s rainfall, overall management is good. Just a few items to improve on. 19 – Yard Waste and Inert Debris area looks good, even though wet conditions present issues. 20 – One load of incoming waste was screened at the working face during this inspection. 21 – Need vegetative cover on landfill. 22 – Digital pictures were taken during this inspection.
