Dear Mr. Hardin and Mr. Stine:

On September 20, 2018, Amanda Freeman, representing the State of North Carolina, Division of
Waste Management Solid Waste Section (Section), inspected the above referenced facility for
compliance with North Carolina solid waste statutes and rules. The facility was found in violation
of the 15A North Carolina Administrative Code 13B .0402 (1), Permit to Operate Attachment 3
conditions, and Approved Operating Procedures. As a result, a Notice of Violation letter was
issued on September 26, 2018. The Section conducted follow-up inspections of the site on
October 10th, 29th and November 2nd, 2018. Due to the fact that the violations noted in the
September 26, 2018 letter were not resolved a Notice of Continuing Violation was issued on
November 5, 2018.

To achieve compliance, the facility was required to properly dispose all waste at an approved
solid waste disposal facility and submit, to the Section, the receipts from the disposal facility. On
November 28, 2018, the Section conducted a follow-up inspection and received the waste
disposal receipts.
