Baker – Scale House operator John College – DWM-SWS-FOB STATUS OF PERMIT: The Permit was issued March 20, 2014 and will expire on March 05, 2019. Submit application for new permit at least six months prior to expiration. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 – The Permit to Operate for this facility was issued on March 20, 2014 and is set to expire on May 05, 2019. Be sure to get your renewal application in no later than December 05, 2018 per your permit, if you have not already submitted it. There was a reminder on the last inspection report dated February 07, 2018. 02 – This facility has 5 Certified Transfer Station Operations Specialist: Gilbert W. Stephen, expires 11/5/18, Stanley Lassiter, expires 11/5/21, Judy M. Baker, expires 5/10/19, Donnell Cooper, expires 11/16/19 and Henry R. Lawrence, expires 11/5/18. 03 – Facility has signage with required information. A new sign was added after the last inspection with No Hazardous Waste included. 04 – Facility has fencing with a locking slide gate at the entrance. 05 – The facility road is constructed of ALL Weather materials. 06 – Facility operating hours are, Monday – Friday 8:30 am to 5:00pm. Saturday hours are, 9:00am to 12:00pm. 07 – Facility is closed on ALL Sundays and the closed Holidays are; New Year’s Day, Martin Luther Day, Good Friday, Memorial Day, Independence Day, Labor Day, Veterans Day, Thanksgiving Day and the day after, Christmas Eve, Day and the day after. 08 – Screening is done at the scale house and at the compactor. Records reviewed. 09 – Incoming load tickets have the required information. 10 – This facility is permitted to receive MSW waste generated within Hertford County and transported to the East Carolina Regional Landfill (Permit Number 0803-MSWLF-1993) located in Aulander, North Carolina in Bertie County. 11 – The tipping floor for the most part is kept in good condition. Area around the compactor could use a little more attention. No Leachate issues noted. 12 – The white goods containing CFC have red X’s on them to indicate the CFC has been removed. Robert Harmon Electric from Powellsville, North Carolina removes the CFC’s. 13 – Scrap tires are laced into a closed top trailer. 14 – Facility also has scrap shingles for recycling. Area was maintained well. 15 – Digital pictures taken during this facility inspection.
