ADDITIONAL COMMENTS:
01 - Signs posted at the entrance have all required information as stated in the permit and this facility is adequately secured by fencing and locking gates.
02 - Interior roadway is constructed of all-weather material and in good condition.
03 - This facility has several Certified Landfill Operations Specialist: Partial listings of operators: Justin Seth Stevenson, expires 02/26/19. Carl Douglas Swain, Jr, expires 12/06/18. Hunter E. Stevenson, expires 12/10/18. Melvin Owens,
expires 08/27/18. Lisa Key, expires 12/18/20. Gregory Hoggard, expires 12/18/20. Facility has more operators that are
not listed on this inspection report. 04 - This facility also has a Certified Manager of Landfill Operations. Brad Gardner, Solid Waste Director, certification
was issued on 06/15/17 and expires on 06/15/20. Certification number: 56116. 05 - Records were reviewed during this inspection. Working face screening and duty station are done. 06 - Weight tickets have proper information.
07 - Last Landfill Gas monitoring records dated 6/18/18. 08 - Ground water monitoring records reviewed. Did not document date. 09 - Landfill Gas wells need information on several tags. Looking for information.
10 - Ground water monitoring wells need information on tags. Facility has all information needed in record book. 11 - Cell #6A has been graded and compacted, edge of waste markers installed and waiting for approval.
12 – Pictures of cell #6A were taken and copies sent to Ethan Caldwell, P.E. - P.G. in the Raleigh Central Office.
13 - The working face appears to be covered at least weekly and more frequently if needed. 14 - Yard Waste and Land Clearing Waste being ground into Mulch during this inspection.
15 – Need to move the windrow back away from the drainage ditch. Must meet 25 feet per permit condition.
16 - Need to instruct Landfill Gas and Groundwater Monitoring Staff to Lock wells. 17 - Overall this facility is being well managed.

Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
