ADDITIONAL COMMENTS
1. Operating hours are 8:00 am to 3:30 pm Monday – Friday.
2. The sign at the facility entrance is correct.
3. The Permit to Operate was issued May 5, 2014 and shall expire May 5, 2024.
4. The permit, operations plan, and records were on site and were reviewed with operator.
5. Certified Landfill Operators:
Brian Thomas, Certified Landfill Operations Specialist, LF-2011069, expires 10/21/20
Dean Brewer, Certified Landfill Operations Specialist, LF-2017035, expires 4/11/20
Christopher Marriott, Certified Landfill Operations Specialist, LF-2016010, expires 3/22/2019
6. The facility maintains semiannual groundwater and surface water monitoring records. Records for sampling event
conducted on October 18, 2017 and April 3, 2018 were verified.
7. Permanent edge of waste markers were in place and being maintained.
8. Edge of waste markers in place. Side slopes are being well maintained, mowing of landfill has been started.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

9. Monitoring wells observed during the audit were locked, capped, and properly tagged.
10. The cover was adequate at the time of the inspection.
11. The access roads to the monofill are being well maintained.
12. Employees working in the landfill receive necessary training and ongoing monthly training in landfill operations.
13. Digital photos were taken during the audit.
