The Permit to Operate shall expire on March 25, 2019. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Tarheel Bark, A Division of Garick, is permitted as a treatment and processing facility, small Type 1 Compost facility and a Land Clearing and Inert Debris Landfill. 2. The LCID Landfill has been approved for excavation and mining for beneficial re-use of the yard waste and wood waste. The facility is not currently excavating or mining the LCID Landfill. The facility is required to notify the Section prior to any excavation work. 3. Please continue working to remove any trees and woody vegetation on the side slopes to allow for proper monitoring and maintenance of the landfill cap. 4. The processing area includes the following activities:  Raw pine bark from a lumber yard is screened to nugget, mini nugget and pine fines.  Hardwood material is brought in already processed and screened into hardwood fines. The hardwood fines are blended with screened fill dirt to make topsoil.  Bulk mushroom compost is brought into the facility and mixed with soil and bagged.  A full bagging operation for the material listed above. 5. No unacceptable waste was observed. 6. Continue to grade the processing area to allow for positive drainage. 7. Take measures to ensure that yard trash which is defined as solid waste resulting from landscaping and yard maintenance such as brush, grass, tree limbs and similar vegetative material are composted and temperature records are maintained demonstrating that temperatures are maintained at or above 131° F for three consecutive days. Please note that leaves are yard trash and must be composted. 8. Reviewed the permit, operations plan and methane monitoring records.
