OBSERVED VIOLATIONS:
1. 15A NCAC 13B .0505(4)(b) states: “Adequate erosion control measures shall be practiced to prevent excessive on-
site erosion.”
During the inspection two areas of active slope erosion were observed on north slope above the sludge ponds. Deposits of
silt observed at the toe of the slope above the sludge ponds. Therefore, PPG is in violation of 15A NCAC 13B
.0505(4)(a) for failure to provide adequate erosion control measure to prevent excessive erosion.
To achieve compliance, PPG must repair all eroded areas and install erosion control measure to prevent excessive
erosion by June 18, 2018.

2. 15A NCAC 13B .0505(6)(a) states: “Within six months after final termination of disposal operations at the site or a
major part thereof or upon revocation of a permit, the area shall be stabilized with native grasses.”
During the inspection a large portion of bare soil was observed between the railroad tracks and the northeast side of the
facility adjacent to the fenced lot. Waste observed in this area consisted of exposed metal, fiberglass, plastics and
electrical circuitry. Mr. Young stated that this condition had existed since at least 2006. Therefore, PPG is in violation
of 15A NCAC 13B .0505(6)(a) for failure to establish native grasses within six months after final termination of
disposal operations.
To achieve compliance, PPG must establish native grasses, repair all eroded areas and install erosion control
measure to prevent silt from leaving the site by June 18, 2018.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Facility ceased all acceptance of solid waste in 1984.
2. Waste in the landfill consisted of fiberglass and some C&D waste.
3. On September 1, 2017 Nippon Electric Glass (NEG) purchased PPG. According to facility staff, as part of the sale
agreement PPG retained responsibility for the closure of the landfill.
4. Gravel pad located on top of the landfill used at times for staging and contractors parking during projects such as
furnace rebuilds.
5. Erosion was observed on the southeast portion of the landfill along the storm water ditch. Some waste (primarily
fiberglass) was observed in the storm water ditch located along the southeast perimeter. Mr. Slaughter and Mr.
Young believe the waste washed into the storm water ditch. Mr. Young did not believe the storm water ditch was
constructed over waste. Remove all waste from storm water ditch and dispose at a properly permitted facility.
Make all necessary repairs to stabilize the storm water ditch to prevent future erosion.
6. A small amount of exposed waste was observed on the southeast side slope of landfill. Remove or cover all
exposed waste.
7. Two waste piles from furnace rebuilds were observed consisting of what appears to be primarily concrete, small
amount of metal, one piece of PVC conduit, slight amount of wood and asphalt located southwest of the gravel
parking area. Remove all waste and dispose at a properly permitted facility.

8. Side slopes on landfill had established vegetative cover.
9. Landfill typically bush hogged once a year.
10. 1st annual groundwater monitoring for 2017 reviewed.
11. An inspection of all monitoring wells was performed to verify proper tags and locks. See table.

12. Please contact Larry Frost by June 1, 2018 to discuss the closure process and documentation of the depth of cover
for the landfill.

13. Mr. John Slaughter with Tetra Tech stated there is no information found confirming the landfill was capped with
two feet of final cover as required by 15A NCAC 13B .0505(3)(c). 15A NCAC 13B .0505(3)(c) states: “After
final termination of disposal operations at the site or a major part thereof, or upon revocation of a permit, the area
shall be covered with at least two feet of suitable compacted earth.”
14. Post-inspection research was conducted to better understand the facility and discuss the closure process. This
research was concluded on May 10, 2018.
