PURPOSE OF SITE VISIT: Pre-operative Facility Compliance Inspection of Phase I Cell 2 STATUS OF PAST NOTED VIOLATIONS: None applicable OBSERVED VIOLATIONS: No violations were observed during this Facility Compliance Inspection; please see the ADDITIONAL COMMENTS section below. ADDITIONAL COMMENTS: 1) This Facility is a Construction and Demolition (C&D) waste Landfill. 2) This Facility is permitted to receive C&D waste generated in Carteret, Craven, Duplin, Jones, and Onslow Counties. 3) This Facility is currently operating in Phase I Cell 1. 4) This Facility is currently only receiving C&D waste from the Maysville C&D Recycling and Recovery Facility, permit #52-02. 5) This Facility does not accept wastes containing asbestos. 6) The Facility access road is of all-weather construction and is being well-maintained at this time. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 5203_INSP_20180507 Page 2 of 3 7) Permanent, highly visible, edge-of-liner markers delineating the waste disposal limits are maintained as required. 8) Construction of Phase I Cell 2 has been completed. Construction appears to have been completed in compliance with the approved construction plan. 9) The outside slopes are properly graded and have been seeded and mulched, however, areas of erosion were observed. *Please repair the eroded areas, and re-seed any areas where the cover crop failed to fully germinate. Continue to monitor the cover crop to ensure that it is established and maintained so that it is suitable to prevent erosion. Any new areas of erosion will need to be graded, seeded, and mulched promptly. 10) Cell 2 may be put into operation upon issuance of the Permit to Operate. 11) The leachate collection system and storage tanks were reviewed. The system appears to be functioning properly and is well maintained at this time. 12) Facility monitoring wells were reviewed.
