PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: NA ADDITIONAL COMMENTS 1. Amanda Freeman, SWS ESS, met with Steve Waters on site at the Highland LCID on April 17, 2018. 2. Purpose of site visit was to conduct a comprehensive inspection. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. No water was observed ponded over waste. Very good. 5. Trees with in the waste foot print had been removed since the last inspection. 6. Edge of waste markers were clearly visible. 7. Ensure, that staff are removing unacceptable waste as it observed. 8. Waste over the top of the landfill was compacted and covered. 9. Waste was exposed on 3 sides of the landfill. Waste on side slopes must be compacted and covered. 10. Excavation, grading, and fill material side slopes must not exceed a ratio of three horizontal feet to one vertical foot (3:1) according to Rule 15A NCAC 13B .0565(3)(j). 11. Solid waste must be covered with a minimum of six (6)-inch-thick of suitable soil cover or to a thickness to cover exposed wastes, whichever is greater. 12. Vegetative ground cover sufficient to control erosion must be established within 30 (thirty) working days upon completion of any phase of LCID development in accordance with Rule 15A NCAC 13B .0566(7) or as addressed in the approved Erosion and Sedimentation Control Plan and permit. 13. Corrective measures are necessary as a result of the findings of the inspection and should be completed within 30 days’ receipt of this inspection report.
