The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The once a year on site meeting. No soil samples taken in 2017. Soil samples shall be taken by the permit
holder in the fall of 2018. Early sampling in the spring is not in accordance with Rule .0840(b) which states
samples shall be taken during the last quarter of the year.
2. The winter rye has been planted but has not come up yet. No land application shall occur this winter unless
the cover crop becomes well established.
3. It appears that the required setbacks are being met but were not measured at this time. No sign of application
outside the permitted boundaries.
4. No sign of heavy or recent application. No odors noted.
5. Field markers appear accurate.
6. The required signs are posted and the access road is adequate.
7. Reviewed site logs, it appears that the required information is being maintained.
8. Discussed use of the cleanout box located at the back of field 1. Review procedures to prevent spillage on the
ground surface during waste transfer.
9. Please provide to this office a key to the gate lock at the entrance.
