PURPOSE OF SITE VISIT:
Comprehensive Inspection
STATUS OF PAST NOTED VIOLATIONS:
None
OBSERVED VIOLATIONS:
1. 15A NCAC 02C .0108(k) states: “All non-water supply wells, including temporary wells, shall be secured with a
locking well cap to ensure against unauthorized access and use.”
Yadkin County is in violation of 15A NCAC 02C .0108(k) for failure to secure landfill gas monitoring wells
against unauthorized access and use.
As part of the facility comprehensive inspection, landfill gas wells (MP-1 through MP-8) were inspected. All
were found to be unsecured.
To achieve compliance, Yadkin County must secure all landfill gas monitoring wells with a locking cap.

2. 15A NCAC 02C .0108(l) states: “All non-water supply wells shall be equipped with a steel outer well casing or
flush-mount cover, set in concrete, and other measures sufficient to protect the well from damage by normal site
activities.”
Yadkin County is in violation of 15A NCAC 02C .0108(l) for failure to equip all non-water supply wells
with a steel outer casing set in concrete.
During this inspection, all 8 landfill gas monitoring wells were observed to be without a steel outer casing set in
concrete.
To achieve compliance, Yadkin County must equip all landfill gas monitoring wells with a steel outer
casing set in concrete.
3. 15A NCAC 13B .1627(d)(1) states, in part: “Following closure of each MSWLF unit, the owner or operator shall
conduct post-closure care. Post-closure care shall be conducted for 30 years, except as provided under
Subparagraph (2) of this Paragraph, and consist of at least the following: (A) Maintaining the integrity and
effectiveness of any cap system, including making repairs to the cover as necessary to correct the effects of
settlement, subsidence, erosion, or other events, and preventing run-on and run-off from eroding or otherwise
damaging the cap system.”
Yadkin County is in violation of 15A NCAC 13B .1627(d)(1) for failing to maintain the integrity of the cap
of the closed MSW landfill.
Much of the landfill cap was not inspected due to the preponderance of briars trees and other woody vegetation
growing on top of the landfill and penetrating the landfill cap, making it impassable. Side slopes were observed to
be steep and wooded, with evidence of erosion in some areas that had resulted in uncovered waste.
To achieve compliance, Yadkin County must perform maintenance to restore the integrity of the closed
MSW landfill cap.
4. 15A NCAC 13B .0543(e)(1) states, in part: “Following closure of each C&DLF unit, the owner or operator shall
conduct post-closure care. Post-closure care shall be conducted for 30 years, except as provided under
Subparagraph (2) of this Paragraph, and consist of at least the following: (A) Maintaining the integrity and
effectiveness of any cap system, including making repairs to the cover as necessary to correct the effects of
settlement, subsidence, erosion, or other events, and preventing run-on and run-off from eroding or otherwise
damaging the cap system.”
Yadkin County is in violation of 15A NCAC 13B .0543(e)(1) for failing to maintain the integrity of the cap
of the closed C&D landfill.
Most of the landfill cap was not inspected due to the preponderance of briars trees and other woody vegetation
growing on top of the landfill and penetrating the landfill cap, making it impassable. Many areas had become
overgrown with kudzu, including the sediment basin located directly south of the transfer station building.
To achieve compliance, Yadkin County must perform maintenance to restore the integrity of the closed
C&D landfill cap.
SUMMARY OF CORRECTIVE ACTIONS REQUIRED:
1. Immediately ensure that all waste is covered at the closed MSW and C&D landfills, and that scrap tires, other
waste, including any windblown litter, are collected from both facilities and disposed of properly.

2. By March 31, 2018, bush hog and clear the entire cap of both the closed MSW and C&D landfills to remove
underbrush, kudzu and small woody vegetation.
3. By March 31, 2018, locate and mark the waste boundaries of both the closed MSW and C&D landfills with
permanent edge-of-waste markers. Note that some markers were observed to be in place during this inspection
and others may be found when the landfill caps are cleared.
4. By April 30, 2018, submit a plan for post-closure maintenance of the closed MSW and C&D landfills that will
address the following:
• maintenance of the required final cover;
• management of trees growing on the landfill caps in a phased approach, including the removal of small
trees and root systems and repair of the cap; monitoring of large trees and removal of dead or diseased
trees.
• prevention of erosion and sedimentation, and the monitoring and maintenance of erosion control devices
and sediment ponds;
• upgrade the existing landfill gas monitoring wells to ensure compliance with 15A NCAC 02C .0108(k)
and (l), as described above in Observed Violations 1 and 2.
The plan will be reviewed by the Solid Waste Section and, once approved, will be implemented by Yadkin
County according to the time table set forth. Please submit the plan to Susan Heim, Environmental
Senior Specialist, NCDEQ-DWM, Winston-Salem Regional Office, 450 W. Hanes Mill Road, Winston-
Salem, NC 27105.
ADDITIONAL COMMENTS
1. The closed C&D landfill and MSW landfill are situated next to each other in the area between the transfer station
and the scale house.
2. Groundwater monitoring takes place semi-annually, as required. The two latest reports available from sampling
events were reviewed. The report covering the March 7, 2017 sampling event indicated exceedances of 2L
standards for 3 metals - chromium, cobalt and vanadium - and for 1 VOC – dichloroethane. The report covering the
August 8, 2017 sampling event indicated exceedances of 2L standards for 4 metals – cadmium, chromium, cobalt
and vanadium – and for 1 VOC – dichloroethane.
3. Methane monitoring is conducted quarterly, as required. The two latest reports available from sampling events were
reviewed. The report for the June 18, 2017 sampling event and the August 8, 2017 sampling event indicated no
exceedances were found.
4. All 9 groundwater monitoring wells were inspected and found to be in good condition. Each was protected in a steel
case, set in concrete, with a hinged lid that was secured by a padlock, and each had a permanently affixed metal
identification tag. All wells were clear of vegetation, and most were easily accessible by vehicle or on foot.
However, MW5S and MW5D were located in a remote spot on the south side of a creek. Crossing the creek and
getting through the brush and brambles to inspect these two wells was difficult. Please ensure that all
groundwater monitoring wells are maintained so that they are clear of vegetation and easily accessible.
5. All 8 landfill gas monitoring wells were inspected. All were easily accessible by vehicle or foot, and all were

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

identified with a permanent metal tag that was secured to each well with wire. None of these wells were protected
within a steel casing set in concrete, nor were they secured. Please ensure that all non-water supply wells are
enclosed in a protective steel case that is set in concrete, and are secured with a locking cap. (See Observed
Violations section of this report for additional information.)
6. Edge-of-waste markers were noted along the western side of the C&D landfill and part of the northern side of the
MSW landfill. However, no edge-of-waste markers were visible on the rest of the perimeter of both landfills. Please
ensure that waste boundaries for both the C&D and MSW landfills are identified with permanent edge-of-
waste markers.
7. To the extent possible, based on site conditions, the landfill caps were inspected. Both the C&D landfill and the
MSW landfill were found to be overgrown with native vegetation, briars and kudzu and were, for the most part,
impassable. One sediment pond was accessible enough to observe from the perimeter roadway, and was also found
to be overgrown with kudzu.
8. Side slopes of the MSW landfill were viewed from the perimeter roadway and found in many areas to be steep and
tree-covered.
9. Some signs of erosion were observed on the side slopes, and it appeared that there were areas of uncovered waste.
10. Scrap tires, other waste and windblown litter were observed near the toe of the MSW landfill in various areas.
Please ensure that all waste, including scrap tires, is collected and properly disposed of immediately. (See
Summary of Correction Actions Required section of this report for additional information.)
11. Please develop a post-closure maintenance plan that will address the issues raised in comments 7 through 10
above. (See Observed Violations section of this report for additional information.)
