Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Inspected vehicle tag#YA99221, tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The permit number is not legible on the passenger side and needs repaired asap. Please send me a photo of the repaired lettering within 30 days. In regards to the rest of the lettering it is correct on both sides & legible, but damaged and will need repaired in the future. 2. The newer vehicle tag#YA131034 was inspected in July. Discussed that the 2018 firm permit application should list the phone numbers that are displayed on both vehicles. 3. Reviewed truck pumping log – ensure that the full physical address including the town name is recorded for each pump job (septic tank & grease traps). 4. Only this truck goes on the land application site & unloads into the storage tank (all waste in the storage tank goes on the fields). A separate log has been maintained for pumping events with this vehicle – begin recording the full physical address for each of these pump jobs. Truck pumping logs shall record the full journey of all waste pumped by any vehicle – from the original location of the waste to the final disposal of the waste.
