PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: NONE OBSERVED VIOLATIONS: NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The site looks better than I have ever seen it and looks like it is being maintained according to the rules. Very good. 2. All edge of waste markers have been installed and easily visible. Make sure that these markers are kept visible. The area for beneficial fill has been marked and insure that these markers stay visible. 3. Ground cover has been established on most of the side slopes except the working face. There are a few areas where the side slopes have gullied. These must be repaired and ground cover established. 4. All slopes have been maintained at no greater than 3:1- very good. 5. The working face appears to be small- very good. 6. All side slopes and top surface have been covered with soil, except for the working face. Insure that adequate soil cover is applied monthly to the working face also, or when the active area reaches one acre in size, whichever occurs first. 7. The facility shall only accept those solid wastes which it is permitted to receive. There was quite a bit of miscellaneous trash, including plastic bottles and other scrap plastic, pieces of unacceptable wood including painted wood, etc. Mr. Gibbs sent an e mail on January 12, 2016 saying that the miscellaneous unacceptable waste has been picked up and disposed of properly. It is suggested that a container be located at the site, so drivers could dispose of unacceptable waste contained in each load. 8. The facility is not open to the public. 9.
