Hilltop Road Red Springs, NC 28377 PARTICIPANTS: Connie Wylie, Emily Cooper STATUS OF PERMIT: Active – permit expires July 14, 2022 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: N/A SEE BACK The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Once a year on site meeting. Discussed past audit & site conditions. No soil samples in 2017. Samples to be taken by permit holder in the fall of 2018. Discussed soil sampling & training classes. 2. Discussed site map & required setbacks. Field markers appear accurate except for a missing post at the corner of the site near the neighboring house (500’ feet setback to the house & 130’ from the centerline of the road). Discussed with Emily by phone & email to reinstall this missing post at the correct location per the setbacks. 3. No sign of land application outside the permitted boundaries. It appears that the approved setbacks are being met but not all setbacks were measured at this time. 4. The winter cover crop is becoming established at this time. If cover crop is sufficiently established then land application may occur this winter. Weed control will be needed in 2018, noted vegetation other than bermuda/rye on site. Discussed with Emily that hay bales sitting on the field perimeter need to be removed when possible. 5. No sign of recent or heavy application. No odors noted. 6. Reviewed site logs (combo log with truck log) and it appears that the required information is being maintained except that since the new permit was issued in July the site has been permitted as two field. The site logs are still showing this site being used as one field. Reviewed field division with Emily on site – next spring begin using the site as two fields per the field rotation in the approved nutrient management plan. 7. The required signs are posted and the access road is adequate. 8. Discussed that for any other firm to discharge on this site an authorization form for 2018 shall be submitted & the discharge events recorded on the site logs.
