The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Once a year on site meeting. Discussed past audits & site conditions. No soil samples taken in 2017. Samples
will be taken by permit holder in the fall of 2018. Discussed soil sampling & training classes.Site logs were
reviewed (combo log with truck log) and it appears that the required information is being maintained.
2. The winter cover crop has been planted but at this time has not become well established over the whole
field. No land application shall occur this winter on this site unless an adequate cover crop is present.
3. Noted scattered heavy/matted application areas. Discussed possible causes & because crop damage can occur
action should be taken to prevent heavy application and to rake loose the current matted spots.
4. Discussed site map & required setbacks. It appears the approved setbacks are being met but were not measured
at this time. No sign of land application outside the permitted boundaries. Field markers appear adequate.
5. The required signs are posted and the access road is adequate. No odors noted.
6. Discussed possibly extending the permitted area slightly to the southwest – field boundary shall be 50’ from the
property line & 200’ from the pond in that direction. A permit modification would be required.
7. Site entrance may be locked in the future – if so please provide this office with a key.
8. Discussed with Emily Cooper that for any other firm to discharge on this disposal field an authorization
form for 2018 shall be submitted & those discharge events shall be recorded in the site logs. Otherwise
only Cooper Septic Tank Service has approval to be on this site.
