with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected pump truck tag #YA126977, VIN# correct – tank appears structurally sound & securely attached.
Outlet covers were on and no drips/leaks were noted. Lettering is correct on both sides except “LLC” is missing
from passenger side – if “LLC” is not on the firm application/permit for 2018 then it is not needed on the trucks.
This vehicle is the only one in use at this time.
2. Also inspected pump truck tag#YA50445, VIN#1H5HJAZNTPH514950, 2500 gallons. Tank structurally sound
& securely attached/outlet covers on.This vehicle is not in use at this time, no insurance & may be added to the
2018 firm permit later in the year (fee for second truck will be needed). The lettering is correct (slightly
damaged but legible) on both sides except the permit number is missing – will need to be corrected on both
sides before truck is added to firm permit/put into use.
3. Reviewed combo truck/land application site log – it appears that required information is being maintained.
