FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. The inspection was conducted at the West Bound NC DOT Rest Area, 19500 Great Smoky Mountains Expressway, Waynesville, NC. 2. The following firm truck was inspected: Vehicle Identification Number (VIN) NC Tag Number Capacity (gallons) 1FTWX33F7XEE63855 HK-6584 250 3. The tank appeared to be structurally sound and bolted to the frame. The permit application states a capacity of 200 gallons; however, previous inspections of this vehicle (with a different company) with the same tank listed a capacity of 250 gallons. 4. Valves and access ports appeared to be secure. Caps were present on the access ports. 5. The truck was properly marked in accordance with the lettering requirements. The application has the firm name as, “A+ Portable Toilets, Inc.” The lettering on the pumper vehicle must match the application. It is suggested that “Inc” be removed from the initial application and further revisions. 6. No leaks were observed. 7. As a reminder, as a new firm in North Carolina, this vehicle cannot be used until a permit is issued by the Solid Waste Section.
