Permit Condition 2 This site shall be operated and maintained in accordance with the nutrient management plan submitted by Keith & Chrisseas Clemons and approved by the Division of Waste Management. It appears that on both fields there is a significant amount of vegetation other than fescue (the approved crop per the nutrient management plan). Land application shall not occur unless there is an adequate cropstand. Reseeding is required if less than 80% coverage with fescue is present. This was previously noted on the audit of 7/27/2017. A notification letter was mailed December 1st in regards to this permit expiring on April 1, 2018. The letter was returned unclaimed. Contacted Chrisseas Clemons and was given the address listed on the front of this report. Notification letter was remailed to this new address. The SLAS permit renewal application, landowner authorization form, nutrient management plan with new signature, site logs for June 14, 2016 until now plus the log cover sheet need to be submitted at this time as it is now less than 90 days prior to the expiration date for this permit. 2. Once a year on site meeting – permit holder unable to meet with me. Requested that I be sent a few pages of the SLAS & truck logs, have not received the logs at this time – send asap. No soil sampling in 2017, sampling will be done by permit holder in the fall of 2018. 3. It appears that the approved setbacks are being met but setbacks were not verified at this time. Field markers appear adequate on both fields. No sign of land application outside the permitted boundaries. 4. No sign of recent or heavy application. No odors noted. 5. Noted some slight ruts on the fields – repair as needed to prevent crop damage. 6. The required signs are posted and the access road is adequate. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
