with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. A notification letter was mailed December 1st in regards to this permit expiring on April 1, 2018. The letter was
returned unclaimed. Contacted Chrisseas Clemons and was given the address listed on the front of this report.
Notification letter was remailed to this new address. The SDTF permit renewal application & landowner
authorization form needs to be submitted at this time as it is now less than 90 days prior to the expiration
date for this permit.
2. The tank appears structurally sound.
3. The approved setbacks are being met.
4. It appears that septage is being transferred to/from the facility in a safe and sanitary manner. No spills or leaks
noted. No vectors or odors.
5. The required 5’ cleared radius around the tank is present at this time.
6. The required sign is posted and the access road is adequate.
