Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Once a year on site meeting. Discussed logs, site map and permit renewal. No soil sampling in 2017, sampling will be done by permit holder in the fall of 2018. Discussed soil sampling, training classes and lime stabilization procedures. 2. Reviewed site map in regards to the required setbacks – setbacks were not measured at this time. Field markers appear accurate. It appears that the approved setbacks are being met. No sign of land application outside the permitted boundaries. Woody area on field 1 has been cut – field markers shall remain posted around this spot as no land application shall occur in this area. 3. Noted spray path on field 1. No odors noted. No sign of heavy application. 4. The fescue appears adequate on both fields at this time. Weed control will be needed in the spring to maintain a good cropstand. 5. The required signs are posted and the access road is adequate. 6. Thank you for your time.
