enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected a third vehicle for the septage firm permit - tag#YA100210, VIN 3ALHCYFE2JDJW9106, 3500
gallons.
2. The tank appears structurally sound & securely attached.
3. Outlet covers were on and no drips/leaks were noted.
4. The lettering is correct on both sides of this vehicle however, the phone number on the vehicle was different
than the phone number listed on the 2017 firm application. Please ensure that the phone numbers displayed on
the vehicles are also listed on future firm applications.
5. Truck logs were not reviewed at this time.
