Untreated medical waste for by-pass TRANSPORTER:

Was: Assured Waste Solutions LLC, Gastonia, NC

Now: Secure Waste Inc., Demascus, MD

TREATMENT FACILITY for untreated medical waste by-pass:

Was: Assured Waste Solutions LLC Medical Waste Treatment Facility (Permit No. 36-15)

Now: Curtis Bay Energy, Baltimore, MD (noted in our SOP OWS-10)

After reviewing the documents in the file system, I have the understanding of the facility operating status and
your request stated in the 01/04/2018 e-mail message that Ozone Waste Solutions, Solid Waste Permit # 4126-
MWP-2014 will
1. Terminate the contract with Assured Waste Solutions, LLC (Permit #36-15) in Gastonia, NC
transporting bypass or untreated medical waste at Ozone Waste Solutions facility in Greensboro, NC
for final treatment and/or disposal.
2. Make new service contract with Secure Waste Inc., Damascus, MD to transport bypass or untreated
medical waste at Ozone Waste Solutions facility in Greensboro, NC for treatment via steam
sterilization at the facility (s) owned by Secure Waste Inc., which is not located in the State of North
Carolina. Bypass or untreated medical waste at Secure Waste Inc. facility will be then shipped to a
incineration facility owned by Curtis Bay Energy in Baltimore, MD.

Since this is business/operation decision made by Ozone Waste Solutions which does not need any permit
action from the Solid Waste Section, your notification is sufficient and adequate. Please place the request
notification and this acknowledgement document to the facility operating record.

This email serves as notification that we have changed companies that transport our untreated medical waste, formerly
Assured Waste Solutions LLC to Secure Waste Inc, Damascus, MD. Assured Waste Solutions LLC Medical Waste
Treatment Facility (Permit No. 36-15) no longer treats the waste via autoclave. Secure Waste Inc transports the
medical waste that cannot be treated via steam sterilization to Curtis Bay Energy – Incinerator in Baltimore, MD.
