I would like to suggest to update the info to the new operations plan when you submit a permit amendment
application for new permit in the future. Thanks Larry to file your request to the document tracking system, I
will review and revise permit language accordingly in the future. Mr. Patrakis will be my consultant on
medical waste matter to handle your any future request as well.

Larry:

We have an Incineration Waste Service Agreement with Curtis Bay out of Maryland to handle our bypass
medical waste.

Since the Ozonator has been down for maintenance, repairs and upgrades, we have been operating as a Transfer
Station with regular medical waste pick ups. Curtis Bay subcontracted our service to Assured Waste Solutions,
LLC (Permit #36-15) in Gastonia, NC. Our billing comes through Curtis Bay but the waste picked up is either
autoclaved at the Assured facility or it is transferred to Maryland for incineration.

During our annual inspection yesterday, Inspector John Patrone suggested I relay this information to you. I will
be documenting in my annual report how much waste was processed at each location. We want to make sure we
are in compliance with all state requirements.
