The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The facility is permitted as being owned and operated by Republic Services of North Carolina, LLC (Republic
Services). However, Republic Services contracts with K. R. Drenth Trucking, Inc. for the operation of the
facility and the transportation of waste for disposal.
2. Sandra Lucas, Scale House Operator, is the only Republic Services staff member working onsite.
3. The facility is permitted to receive municipal solid waste that is generated within Forsyth, Alamance,
Alexander, Alleghany, Cabarrus, Caswell, Chatham, Davidson, Davie, Guilford, Iredell, Randolph,
Rockingham, Rowan, Stokes, Surry, Wilkes, and Yadkin Counties, North Carolina.
4. Waste from the facility is transported to Uwharrie Regional Landfill (Permit # 62-04) for final disposal.
Alternate disposal sites for waste leaving the facility are Republic Services’ Upper Piedmont Regional Landfill
(Permit # 73-04), and Foothills Regional Landfill (Permit # 14-03).
5. A copy of the Permit to Operate and the approved Operation Plan (revised April 2017) were available and
reviewed.
6. Records of daily tonnage received at the facility are kept by hand on a wall calendar by the scale house
operator. A review of daily tonnage records for 2017 found that between 350 and 520 tons of MSW are
accepted at the facility daily. Monthly tonnage reports were not available at the facility, but were provided
digitally by Mr. Tabaczynski after the inspection. These digital records indicated that 44,849 tons of MSW were
received during the current fiscal year, from July 1 through November 30, 2017.
7. A review of the waste screening records dated January 1, 2017 through December 8, 2017 revealed that a
random load is inspected at least daily, as specified in the approved Operation Plan. Mr. Prestandrea stated that
approximately 30 to 50 loads of waste are received during each operating day, and he estimates an average of
20 trailer loads are transferred out of the facility daily.
8. Training records were reviewed, and the following personnel were certified Transfer Station Operations
Specialists:
a. Sandra Lucas – Republic Services – expires 7/15/2018;
b. Mark Tabaczynski – Republic Services – expires 1/1/2021;
c. Daryl Stratton – Drenth Trucking – expires 2/27/2018;
d. Joseph E. Prestandrea – Drenth Trucking – expires 4/5/2020;
e. Lamar Crawford – Drenth Trucking – expires 6/13/2020.
9. Digital copies of facility monthly inspection reports, maintained and stored at the corporate offices, were
forwarded via email by Mr. Tabaczynski after the inspection. The records provided covered the period July 1
through November 30, 2017. A review of these inspection reports revealed only minor maintenance issues, such
as siding repairs and potholes. These issues are caught and dealt with in a timely manner as a result of this
procedure.
10. The approved Operation Plan (April 2017) specifies elements of the facility that are required to be inspected
monthly, such as transfer trailers, push walls, and the leachate collection system. However, some of these
elements are not included in the inspection log checklist. Mr. Tabaczynski stated that these elements are indeed
inspected as required, but that the checklist is a generic form used at Republic facilities and, therefore, has not
been altered to reflect the requirements and conditions of this facility. It is recommended that the monthly
inspection checklist be revised to reflect the actual inspection requirements. Please maintain copies of
monthly inspection logs at the facility.
11. During this inspection, the partially-loaded transfer trailer (identified with a label as T-422) parked in the
loading bay was observed to be leaking liquid from the trailer floor. Although the leachate was flowing to the
trench drain and into the leachate collection system, the question was posed to Mr. Prestandrea as to what would
be done to prevent leachate from leaking outside the collection system onto the facility grounds and ultimately
onto the public road after the trailer was fully loaded. Mr. Prestandrea stated that he had no means of emptying
the trailer, so it would leave the facility in the condition observed when it was full.
12. Four empty transfer trailers were staged to the rear of the transfer station building at the northeast end of the
facility parking area. Upon inspection, two of these four trailers (marked with labels as T-399 and T-214) were
found to be out of compliance with 15A NCAC 13B. 0105(c)(4), having holes, cracks and damage that would
permit waste to escape the trailers. Mr. Prestandrea stated that, in all likelihood, he would use those trailers to
transport waste before they could be repaired.

13. Following the inspection, David Haywood, Maintenance Manager, K R Drenth Trucking, Inc., provided
confirmation that all three trailers in question had been removed from service for repair, and that repairs had
already been completed on two of the trailers.
14. Although transfer trailers are owned by a contracted service provider, the trailers are under the control
of this permitted facility. Republic Services is the permit holder, owner and operator of this facility. As
such, it is the responsibility of Republic Services to ensure that all transfer trailers used for transporting
waste from the facility for disposal are maintained in a leak resistant condition, and that all cracks, holes
and damage that would allow waste to escape the trailer and be released into the environment are
repaired prior to use, as required by 15A NCAC 13B .0105(c)(4).
15. The entire facility, including the transfer station area, the scale house area and the parking and trailer/container
storage areas, was observed to be clean and well maintained.
16. Site security is maintained by means of a chain link fence that surrounds the entire facility. Locking entry and
rear gates are secured when the facility is not in operation to prevent unauthorized access.
17. Some windblown litter was observed outside the chain link fence line that surrounds the facility on the north
side of the entrance. Ensure that all windblown litter is collected at the end of every operating day and
returned to the transfer station for proper disposal.
18. Ms. Lucas stated that the facility has experienced problems with windblown litter from Abbey Green (Permit
34-24), the neighboring facility to the west. She added that she has spoken with Abbey Green’s manager about
the problem on many occasions.
19. Ensure that the contact information on the sign at the facility entrance is updated and that it includes all
of the required information as specified in the permit to operate. This will be verified during next
inspection.
20. The entire site is paved and the access roadways are well-maintained.
21. The facility is designed to allow sufficient room for traffic to enter, maneuver and exit safely. Weighing,
dumping and exiting trucks were observed during this inspection. The transfer station equipment operator
directed each truck, and maneuvering appeared to be accomplished with no difficulty.
22. Empty roll-off containers are stored in the large lot to the rear of the transfer building.
23. The tipping floor was inspected and generally appeared to be in good condition. However, near the center of the
tipping floor, a sizeable hole in the concrete was observed. Mr. Prestandrea stated that he had no knowledge of
this damage to the tipping floor prior to the inspection. Mr. Tabaczynski was notified of the damage following
the inspection, and he conducted his own inspection of the tipping floor that same day. He also provided
information about the extent of the damage and the steps he had taken to initiate repairs, adding that repairs
would be completed as soon as the temperatures rose above freezing to enable the patch to cure properly.
24. The leachate collection system for the transfer station operation is comprised of an opening in the loading bay
wall that allows water to drain from the tipping floor, which is sloped so that all liquids are directed to the
northern corner of the building where the opening is located. The wall of the loading bay directly under this
drainage opening was observed to have a significant build-up of waste and leachate on it. Mr. Prestandrea stated
that the loading bay is swept out daily, but not washed down. He added that Republic Services pressure washes
the building at least annually.
25. Waste was observed to have accumulated on top of the beams of the rear wall of the loading bay.
26. Ensure that the entire transfer station, including the loading bay, is maintained in a clean, sanitary
condition at all times and is cleaned at least daily, as specified in the Permit to Operate.
27. Floor drains in the loading bay send liquids to the oil-water separator, from which accumulated solids are
removed as necessary by Safety Kleen. The separated liquid then runs directly to the sanitary sewer system. No
pre-treatment of the leachate is required. Floor drains were observed to be free of accumulated litter and
appeared to be functioning properly.
28. The waste deflector in the loading bay was in place and functioning to help direct all waste from the loader
bucket into the trailer.
29. Two water hoses, located to the right of the entry to the tipping floor and in the loading bay, are used for clean-
up and are also available for fire suppression. Fire extinguishers are located with the water hoses and at other
strategic locations around the facility. The Forsyth County Fire Marshal inspects the facility annually.
30. Following the inspection on January 4, 2018, Mr. Tabaczynski notified the Solid Waste Section that he
anticipated having waste left on the tipping floor at the close of business. He explained that the Uwharrie

Environmental Landfill (Permit # 6204) had been affected by winter weather and had postponed the start of the
operating day. As a result, the Drenth truck drivers stationed in Montgomery County had not begun to make
runs at their normal time, so were transferring fewer loads from the transfer station to the landfill. In addition,
several other facilities had redirected their waste to the transfer station due to winter weather, so there was more
waste than usual being accepted at the transfer station. Please note moving forward, that while the Section
will grant the authorization to store waste on the floor outside of normal business hours in emergency
situations, it is the responsibility of the owner and/or operator to have contingency plans for the
acceptance, management, and ultimate disposal of waste in all emergency situations. The Section will
only authorize such action when said contingencies have been demonstrated to be exhausted, which
doesn’t appear to have occurred in this situation as disposal locations other than Uwharrie were readily
available. The Section further reserves the right to require all waste be stored in leak resistant containers
or trailers rather than on the tipping floor should it be deemed more protective of human health and the
environment given the particular circumstances.
