Life of Site permit issued on December 29, 2016. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS; 1 - Signs posted at the entrance have all required information as stated in the permit and this facility is adequately secured. 2 - Interior roadway is constructed of all-weather material and in good condition. 3 - This facility has several Certified Landfill Operations Specialist: Partial listings of operators: Justin Seth Stevenson, expires 02/26/19. Carl Douglas Swain, Jr, Expires 12/06/18. Hunter E. Stevenson, expires 12/10/18. Melvin Owens, expires 08/27/18. Facility has more operators that are not listed on this inspection report. 4 - This facility also has a Certified Manager of Landfill Operations. Brad Gardner, issued on 06/15/17 and expires on 06/15/20. Certification number: 56116. 5 - Records were not reviewed during this inspection. 6 - Be sure the working face is being covered at least weekly and more frequently if needed. Some uncovered, excess waste away from the working face in need of covering. 7 - Ray Williams (DWM) noticed a small area of smoldering (smoke) on the East side of the working face. Staff was directed to this area and started to work on this small area to ensure no major fire was going to occur. 8 - John College will conduct a follow up inspection when he is notified to inspect the Closed MSWLF, cell #1 when repair is done. 9 - Still have some plastic bags getting into the Yard Waste piles. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
