Former Swift Creek Coal Combustion By Product

Introduction
ERM NC, Inc. (ERM) has been contracted by ReUse Technology, Inc. (ReUse) to implement the
Groundwater Corrective Action Plan for the Former Swift Creek Coal Combustion By Product
(CCB) Structural Fill, CCB0057 approved by NCDEQ – Division of Waste Management on
March 12, 2015. As requested by NCDEQ, ERM will provide monthly updates to summarize
site activities including any changes in conditions as the project progresses.
ERM has subcontracted Earnhardt Grading, Inc. (Earnhardt) of Lowesville, North Carolina to
provide site preparation, excavation, grading, geosynthetic installation and final site
stabilization. ERM and Earnhardt have been on the Swift Creek site full time beginning April
24, 2017 for construction activities. ERM has compiled weekly update status reports that are
attached for reference.
Summary of Activities
Since the previously issued Monthly Project Update dated October 24, 2017, ERM and
Earnhardt Grading have remained onsite full time conducting corrective action plan activities.
These activities included:
Removal of CCB materials from the north end (Phase 2 area) of the project site.
Placing CCB materials on the south end raised floor (Phase 1) area of the project site.
Achieving final proposed grades for the south end CCB materials closure.
Installation of 40-mil Linear Low Density Polyethylene (LLDPE) liner over the entire
CCB closure footprint.
Installation of double sided 6-ounce drainage geocomposite above the 40-mil liner.
Installation of liner system anchor trench, perforated pipe, anchor trench drains and
backfilling with washed stone (95% complete).
Importing of offsite soils for protective final cover over the liner system.
Maintaining erosion and sedimentation control measures per NCDEMLR – Division of
Land Quality approved plans (approved March 31, 2017).
Surveying as-builts for removal of CCB materials and finished closure footprint grades.
At the end of this seventh month of construction activities, Earnhardt has completed 100% of
the liner system installation. Due to heavy rains in the vicinity of the project, there have been
several days lost to production due to weather and/or wet conditions.

Site Observations
To date there have been no visual indications of dust migration offsite.
Erosion and sedimentation control measures are functioning appropriately and there
have been no observations of offsite sediment releases.
Monitoring wells have been marked with construction fence and maintained throughout
construction activities.

Weekly Update –October 25, 2017
Key Items Completed Last Week:
Project Related
1. Earnhardt Grading and ERM remain on site full time since 4/24/17.
2. CCR (coal combustion residuals) materials were transported to the southern stockpile
for final placement between Wednesday 10/18/17 and Monday 10/23/17 including
Saturday and Sunday. Good progress each day.
3. The team was not able to transport ash Tuesday 10/24/17 due to 0.75” of rain the night
before. The team worked on required equipment maintenance and repair.
4. No damage occurred to the erosion and sedimentation controls during the rain event.
5. Progress Plan and photo log are attached for reference.
Two (2) Week Look Ahead (10/24/17 – 11/08/17):
6. Earnhardt and ERM will remain on site full time.
7. Earnhardt will continue to move Phase 2 CCR materials to the southern stockpile as final
placement. Good weather is forecasted through Monday the 30th.
8. Final stockpile grading and preparation for geosynthetic liner system.
9. Geosynthetic liner is scheduled for delivery the week of November 6th.
10. ERM will continue to update Full Circle concerning site activities as needed.

11. NCDEQ; DEMLR – Land Quality – Thad Valentine was notified on May 12, 2017 that
erosion and sedimentation control measures are in place for an onsite meeting. Thad
replied that he would stop by next time he was in the area. No visit has occurred yet.
12. NCDEQ Monthly Update was submitted on October 24, 2017. No questions or concerns
have been received from NCDEQ.
Schedule
Currently, approximately 85% of CCR materials have been transported for final
placement. Delays have been weather related due to above average rainfall in this
region of North Carolina.

1. Earnhardt Grading and ERM remain on site full time since 4/24/17.
2. CCR (coal combustion residuals) materials were transported to the southern stockpile
for final placement between Wednesday 10/25/17 through Friday 10/27/17 and
Tuesday 10/31/17. Good progress each day.
3. The team was not able to transport ash Monday 10/30/17 due to 1.7” of rain the day
before.
4. No damage occurred to the erosion and sedimentation controls during the heavy rain
event.
5. Progress Plan and photo log are attached for reference.
Two (2) Week Look Ahead (11/1/17 – 11/15/17):
6. Earnhardt and ERM will remain on site full time.
7. Earnhardt will continue to move Phase 2 CCR materials to the southern stockpile as final
placement. Good weather is forecasted through Wednesday the 8th.
8. Final stockpile grading and preparation for geosynthetic liner system.
9. Importing of off-site soil for final cover.
10. Geosynthetic liner is scheduled for delivery the week of November 6th.
11. Geosynthetic liner is scheduled for installation starting November 13th.
12. ERM will continue to update Full Circle concerning site activities as needed.

1. Earnhardt Grading and ERM remain on site full time since 4/24/17.
2. CCR (coal combustion residuals) materials were transported to the southern stockpile
for final placement between Wednesday 11/1/17 through Tuesday 11/7/17 including
the weekend. Good progress through Monday 11/6/17.
3. The team was unable to operate Tuesday (11/7/17) afternoon due to rain. The team had
difficulty moving material in the morning due to slick conditions.
4. Geo-composite material and liner arrived Monday and Tuesday (11/6-7/17) and was
inventoried.
5. Progress Plan and photo log are attached for reference.
Two (2) Week Look Ahead (11/8/17 – 11/22/17):
6. Earnhardt and ERM will remain on site full time.
7. Earnhardt will move the remaining Phase 2 CCR materials to the southern stockpile as
final placement.
8. Final stockpile grading and preparation for geocomposite liner system.
9. Importing of off-site soil for final cover.
10. Geocomposite material and liner is scheduled for installation starting Monday
November 13th.
11. ERM will continue to update Full Circle concerning site activities as needed.

12. NCDEQ; DEMLR – Land Quality – Thad Valentine was notified on May 12, 2017 that
erosion and sedimentation control measures are in place for an onsite meeting. Thad
replied that he would stop by next time he was in the area. No visit has occurred yet.
13. NCDEQ was advised of ash within 10’ of known utilities. NCDEQ concurred that the
best course of action is to leave the CCR material in place because of the proximity of the
utilities.
Schedule
Currently, approximately 95% of CCR materials have been transported for final
placement. The liner installation is scheduled to begin Monday 11/13/17. Delays have
been weather related due to above average rainfall in this region of North Carolina.

Weekly Update –November 15, 2017
Key Items Completed Last Week:
Project Related
1. Earnhardt Grading and ERM remain on site full time since 4/24/17.
2. CCR (coal combustion residuals) materials were transported to the southern stockpile
for final placement on Wednesday 11/8/17 through Thursday 11/9/17, and Monday
11/13/17 through Tuesday 11/14/17.
3. On Thursday 11/9/17 during ash transport activities an excavator rolled over during
operation. The excavator operator was taken to the Fastmed clinic to get checked. The
operator was fine. The excavator was turned back on its tracks on Friday. Minor damage
occurred to the excavator including a broken mirror. No fuel spillage was observed. A
small amount of gear oil was observed and cleaned up using an onsite spill kit.
4. The liner crew arrived on-site Monday morning and prepared to install liner. Liner
installation began on Tuesday 11/14/17. Fifteen panels of liner were installed and
welded on Tuesday for approximately 1.5 acres of installation.
5. The team had difficulty moving ash on Monday 11/13/17 due to 0.6” of rain that the site
received Sunday night. The weather also delayed the liner crew due to difficulty moving
around the site.
6. The site began receiving backfill material for the liner cover on Tuesday 11/14/17.
7. Progress Plan and photo log are attached for reference.
Two (2) Week Look Ahead (11/15/17 – 11/29/17):
8. Earnhardt and ERM will remain on site full time.
9. Earnhardt will move the remaining Phase 2 CCR materials to the southern stockpile for
final placement.
10. Final stockpile grading and preparation for geosynthetic liner system.
11. Importing of off-site soil for final cover.
12. Liner installation should be complete by Thanksgiving pending favorable weather
conditions.
13. ERM will continue to update Full Circle concerning site activities as needed.

14. NCDEQ; DEMLR – Land Quality – Thad Valentine was notified on May 12, 2017 that
erosion and sedimentation control measures are in place for an onsite meeting. Thad
replied that he would stop by next time he was in the area. No visit has occurred yet.
15. NCDEQ - The monthly update to NCDEQ is due on 11/24/17. ERM will be preparing
and submitting the update on or before the due date.
Schedule
Currently, approximately 98% of CCR materials have been transported for final
placement. The liner crew has started and are scheduled to be finished by Wednesday
11/23/17. Delays have been weather related due to above average rainfall in this region
of North Carolina.

Weekly Update –November 29, 2017
Key Items Completed Last Week:
Project Related
1. Earnhardt Grading and ERM remain on site full time since 4/24/17.
2. CCR (coal combustion residuals) materials were transported to the southern stockpile
for final placement on Wednesday 11/15/17 and Thursday 11/16/17. All CCR transport
and final ash grading for the stockpile was completed on 11/16/17.
3. The liner crew arrived on-site Monday morning and prepared to install liner on Tuesday
11/14/17. All liner material was installed by 11/28/17 and the geocomposite is
approximately 90% complete.
4. Earnhardt continued receiving backfill for the final cover of the liner on 11/15/17
through 11/20/17. The site did not receive fill material on 11/21/17 through 11/26. Fill
material import continued on Monday 11/27/17.
5. A work stoppage occurred starting Monday 11/20/17 afternoon through Sunday
11/26/17 due to a safety incident that occurred on Monday 11/20/17 resulting in a
fatality. This incident remains an open investigation with OSHA led from the Raleigh,
NC office. Following an OSHA site visit on 11/21/17 and a safety management review,
work resumed 11/27/17.
6. The anchor trench excavation was completed on Tuesday 11/28/17. Approximately 80%
of the anchor trench drain line and gravel have been installed as of Tuesday afternoon
11/28/17.

Two (2) Week Look Ahead (11/29/17 – 12/13/17):
8. Earnhardt and ERM will remain on site full time.
9. Earnhardt will complete the anchor trench.
10. The final cover soil will be added following completion of the anchor trench.
11. Importing of off-site soil for final cover.
12. ERM will continue to update Full Circle concerning site activities as needed.

13. NCDEQ; DEMLR – Land Quality – Thad Valentine was notified on May 12, 2017 that
erosion and sedimentation control measures are in place for an onsite meeting. Thad
replied that he would stop by next time he was in the area. No visit has occurred yet.
14. NCDEQ - The monthly update to NCDEQ will be submitted this week following
completion of liner installation. ERM also notified NCDEQ of the onsite incident that
occurred on 11/20/17 via a telephone conversation with Jackie Drummond on
11/28/17.
Schedule
Currently, all CCR materials have been transported for final placement and covered
with liner. The liner crew is scheduled to finish 11/29/17. Following completion of the
anchor trench, the final protective cover will be installed and final seeding and
stabilization will occur following the final cover efforts. Delays have been weather
related due to above average rainfall in this region of North Carolina.
