The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The permitted facility consists of four 18,000-gallon and one 36,000-gallon in ground concrete tanks with a
total capacity of 108,000 gallons for the storage of untreated and treated septage. Also observed was a
dewatering box.

2. The facility is permitted for the storage and treatment of grease trap pumpings.

3. Wastewater from this facility is permitted to be discharged to the Archie Elledge WWTP. Solids from the
treatment process are permitted to be disposed at the Haynes Mill Road Landfill (Solid Waste Permit 34-02),
Winston-Salem, Forsyth County, NC. Solid waste material is disposed at a permitted composting facility.

4. Appropriate signage was present.

5. No objectionable odors were observed outside of the facility.

6. The permitted tanks appeared to be structurally sound and no leaks were observed.

7. The box clean-out area consisting of a concrete pad draining to a buried septage detention tank observed in
previous inspections has been taken out of service and not used.

8. A polymer used in the dewatering process was observed disposed of on the ground. All waste must be disposed
of in a proper manner. An employee was directed to clean up the material during the inspection.

9. Operational records were observed at the office and appeared to be in order.
