PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Reviewed semi-annual groundwater (April 2015, October 2015, April 2016, October 2016 & April 2017) and monthly methane monitoring reports continue to be performed by facility staff. 2. A gate has been provided at the entrance to the facility to prevent unauthorized entry. The gate was shut and locked at the time of the inspection. 3. Edge of waste markers were in place and visible along the perimeter of the landfill. 4. The landfill was last mowed during the summer months. An inspection of the landfill to inspect for any erosion issues, depressions and leachate seeps was not performed due to the tall established vegetative cover. Facility staff was on-site mowing during the inspection. Upon completion, an additional inspection will be performed of the landfill. 5. An inspection of all monitoring wells was performed to verify proper tags and locks. 6. Temporary disaster debris site (DS55-005) is located in this area, and is still available for use according to facility staff. 7.
