Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Once a year on site meeting. Discussed past audits & site conditions. No soil samples taken at this time. Soil samples shall be taken by permit holder in the fall of 2018. Reviewed combo logs which appears to show the required information for both the truck pumping log & the site logs – however different field names are being used other than the field names listed on the site map & permit. 2. No sign of land application outside the permitted boundaries. No sign of recent or heavy application. No odors noted. 3. The required signs are posted however the sign on field 3 is faded and the sign on field 2 is not visible when the gate is open (which it was during this visit). The access road is adequate. 4. It appears that the approved setbacks are being met – were not measured at this time. Field markers appear adequate (an additional marker was posted on field 2 since the last visit). 5. The fescue appears improved on field 2. Please continue efforts to control weeds & vegetation other than fescue in the coming year.
