The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Mr. Cheek was in the process of cleaning up the tank area during this visit. Instructed Mr. Cheek to remove
the excess vegetation at the rear of the tanks to establish the required 5’ cleared radius around the tanks.
2. The third tank from the left (when facing the tanks) has rust holes on the right side – was informed by
Aaron Cheek that this tank is no longer in service. This tank shall be emptied of all waste and shall not be
used for septage detention unless it is adequately repaired. The other remaining (4) tanks appear
structurally sound.
3. It appears that septage is being transferred to/from the facility in a safe and sanitary manner. No spills or leaks
noted. No apparent vectors or odors.
4. The required signs are posted and the access road is adequate.
5. The approved setbacks are being met. No physical change in facility.
6. Discussed & provided information for the permitting process for a septage treatment/dewatering facility.
