OBSERVED VIOLATIONS: N/A SEE BACK The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Once a year on site meeting with permit holder. Discussed past audits and site conditions. Discussed soil report from samples taken in 2017, higher lead levels on fields 2 & 10. Permit holder has recently taken soil samples and will review lead levels on new report. 2. Reviewed site map & setbacks with permit holder. Inspected field markers (additional posts have been placed at fields 6 & 7) – field markers appear accurate. It appears that the approved setbacks are being met at this time. 3. Discussed/reviewed the approved crop for each of the 16 fields per the approved nutrient management plan. Repair work (fescue) has been done to part of field 9. Mr. Woodall stated that the bermuda fields (13, 14 & 16) are not used over the winter and therefore a rye crop is not planted. 4. No sign of land application outside the permitted boundaries. No sign of recent or heavy application. No odors noted. 5. The required signs are posted and the access road is adequate. 6. Thank you for your time.
