The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Once a year on site meeting. Reviewed site logs – it appears that the required information is being maintained.
No soil sampling in 2017, sampling will be done by permit holder in the fall of 2018.
2. Reviewed past audits & site conditions. The fields have been cut but the hay is still on field 2 at this time. The
winter cover crop has not yet been planted. Noted areas of thin bermuda & weeds. Contact Georgia Love early
next year for advice regarding weed control & improving the bermuda.
3. Reviewed site map & the required setbacks. The woodline needs to be cut back to reestablish the 10 foot
setback and also to ensure that the field markers are not hidden by vegetation. Ensure the 50’ setback is
maintained along the property line on the north side – also a 50’ setback from any food crop for human
consumption on this adjacent field (last crop was cotton). Discussed where the property lines are located -
currently it appears that these markers are accurate. Measured the distance from the railroad tracks to the east
side field markers at approx.. 68’, 64’ & 60’ – this train track has been abandoned due to storm damage.
4. No sign of recent or heavy application. No odors noted. No sign of application outside the permitted boundaries.
5. The required signs are posted, but the access road will need maintenance in the near future.
