administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected two vehicles.
Tag#YA60197, VIN GDM7H1J6RJ523707 – repair work is planned for the lower back portion of the
tank/truck which has rusted, but is not leaking at this time. The lettering is correct on both sides of this vehicle,
but is slightly damaged. Outlet covers were on and no drips/leaks were noted.
Tag#YA023317, VIN1M2AY04YXJMD03800 – the tank appears structurally sound & securely attached. The
lettering is correct on both sides of this vehicle. Outlet covers were on and no drips/leaks were noted.
The third vehicle listed on the 2017 firm application is not currently in use and was not inspected at this time.
2. Reviewed the truck pumping log – discussed the required information to be maintained, the discharge location
for each pumping event was not clearly noted – begin recording the exact discharge location.
3. Mr. Locklear recently attended a training class in Greensboro and has also received the firm renewal packet.
