Compliance Inspection Report
Blaylock Grading LCID Landfill

Dear Mr. Blaylock,
On April 26, 2017, Liz Patterson, representing the State of North Carolina, Division of Waste
Management Solid Waste Section (Section), inspected the above referenced facility for
compliance with North Carolina solid waste statutes and rules. David Stephens was present
and represented Blaylock Grading LCID Landfill during this inspection. The following violation
was noted:
A. 15A NCAC 13B .0566(4), “Adequate soil cover shall be applied monthly, or when the
active area reaches one acre in size, whichever occurs first.”
Blaylock Grading LCID Landfill is in violation of 15A NCAC 13B .0566(4), in that at the time of this
inspection, uncovered waste was exceeding one acre and had not been covered as required by rule.
Based upon the foregoing, Blaylock Grading LCID Landfill shall come into compliance within 30
days of receipt of this Notice of Violation with all requirements of the regulations in 15A NCAC
13B .0566(4) by completing the following:
1) Upon receipt of this Notice of Violation, uncovered waste needs to be covered
within 30 days of receipt of this Notice of Violation
2) Operator will continue to keep the LCID working face as small as possible with a ½ acre
maximum size to comply with #7 of the approved operations plan.
The violation(s) listed above were observed by Section staff and require action on behalf of
Blaylock Grading LCID Landfill in order to come into or maintain compliance with the Statutes,

Rules, and/or other regulatory requirements applicable to this activity. Be advised that pursuant
to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each
violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9
of Chapter 130A of the N.C. General Statutes. Further, Blaylock Grading LCID Landfill may also
be subject to enforcement actions including penalties, injunction from operation of a solid waste
management facility or a solid waste collection service and any such further relief as may be
necessary to achieve compliance with the North Carolina Solid Waste Management Act and
Rules.
In addition to the violation noted above, please note that items #12, #14, and #16 on the
inspection report required additional action on behalf of the facility in the next 30 days to ensure
compliance is maintained at this facility. Should these items not be addressed in that time frame,
the facility will be subject to future enforcement action(s).
Please keep me informed of your progress in this matter. Solid Waste Section staff will conduct
a follow-up inspection to verify that the facility has completed the requirements of this Notice of
Violation.
