with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Once a year on site meeting. Received a copy of the site logs & discussed format – need time of discharge on
fields and do not record other types of waste as this negatively affects “total amounts”. No soil sampling in
2017, sampling will be done by permit holder in the fall of 2018.
2. Discussed past audits & site conditions. Croptstand appears adequate.
3. Reviewed site map & discussed required setbacks. Excess vegetation needs to be removed from around some
field markers and the 5’ setback from the woodline needs to be established in some areas.
4. No sign of land application outside the permitted boundaries.
5. No sign of heavy or recent application. No odors noted.
6. The required signs are posted and the access road is adequate.
