Facility permit 51-03 is a combination of facility permits 51-01 and 51-02. PTO MSW – Phase 4A: May 27, 2015, PTC C&D Area 2, Cell 2 and PTO: May 27, 2015 PTO MSW and CDLF expires September 28, 2019 PURPOSE OF SITE VISIT: Partial inspection STATUS OF PAST NOTED VIOLATIONS: NONE OBSERVED VIOLATIONS: NONE FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 5 ADDITIONAL COMMENTS 1. Rick Proctor with Johnston County contacted Liz Patterson, Environmental Senior Specialist with NCDEQ, Division of Waste Management on Tuesday, August 29, 2017 to self-report a leachate release at Johnston County Landfill. The leachate was first noticed on Tuesday morning, August 29, 2017. 2. The service area for the MSWLF and C&DLF includes all 100 counties in North Carolina. 3. The hours of operation are M-F, 8:00 am – 4:45 pm, Sat. 8:00 am – 3:30 pm, Sun. – Closed. 4. The facility has proper signage. 5. The facility is properly secured with a locked gate. Personnel are onsite during operating hours. 6. The facility has a back-up generator for leachate pumping in case of power outages. 7. The landfill gas generator is online and sending to the grid. The generator is located to the northeast side of the office. The generator is privately owned. 8. Permit, operations plan, and site plan were readily accessible and reviewed with the operator. 9. Waste screening records were reviewed and is being conducted on about one percent of the waste stream. Waste Screening forms current through 4/18/17. 10. Cover logs noting the date and time of placement were reviewed and are being maintained properly. 11. Previously, the C&D, Phases 1-4 and 4A, and Phase 5 units were each reported separately. However, the monitoring network was revised in the updated Water Quality Monitoring Plan(WQMP)2, included in the 2014 Permit Renewal Application3, which established a single report for the facility rather than separate reports for each landfill unit. 12. The facility maintains semiannual groundwater and surface water monitoring records. Records for sampling events conducted on June 13-16, 2016 and November 8-9, 2016 were verified. Semi-annual groundwater monitoring completed by Smith Gardner, Inc. MSWLF 13. The working face at the MSW landfill appeared to be kept small and appeared to be well compacted with no unacceptable wastes observed. 14. No evidence of leachate breakouts was observed during the inspection. 15. The intermediate cover at the facility appeared in good condition and well applied. 16. Side slopes are well maintained. 17. Permanent edge of waste markers were in place and being maintained. 18. Perimeter road is being well maintained. 19. Windblown waste was being well managed. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The leachate release originated from a faulty pipe coupling approximately 30 feet east of the C&D dewatering manhole and traveled adjacent to the road approximately 85 feet west toward the drain under the road. There was no visual evidence leachate entered the drain culvert. Following discovery of the release the coupling was repaired and approximately 100 cubic yard of wet soil was removed and placed into the MSW Landfill. Area of leachate release 21. Leachate Release Response Plan submitted to NCDEQ-Division of Waste Management on September 8, 2017. Plan was approved on September 11, 2017 under the condition that the soil samples are analyzed for total phosphorus, sulfate, and nitrate in addition to VOCs and metals 22. The working face at the C&D landfill was kept small and appeared to be well compacted. 23. No unacceptable wastes observed in the working face. 24. Side slopes are being stabilized with mulch and are well maintained. 25. Permanent edge of waste markers were in place and being maintained. 26. Perimeter road is being well maintained. 27. Windblown waste was being well managed YARD WASTE 28. Yard waste is being well managed. 29. No incidental waste (plastic bags, etc.) was observed. 30. Digital pictures were taken during the inspection. CLOSED MSWLF 31. There was no visible ponding of water over waste. 32. Permanent edge of waste markers were in place and being maintained. 33. Perimeter road is being well maintained. 34. Good vegetation established overall. 35. Area on north side below the liner where erosion was occurring has been repaired and covered with layer of mulch to help establish vegetation. 36. Continue to keep vegetation properly mowed to allow for routine inspections and address any issues noted as soon as practical after detection to reduce or eliminate potential compliance issues. 37. No evidence of leachate breakouts was observed during the inspection. 38. Monitoring wells were accessible, properly labeled, and locked.
