Consultants STATUS OF PERMIT: Permit To Operate (PTO) issued April 10, 2017 Permit To Construct (PTC) issued in conjunction with PTO PTO expiration date January 19, 2026 PURPOSE OF SITE VISIT: Partial Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS On October 3, 2017, John Patrone and Ming-Tai Chao met with Daniel Moore, Mike McFeeley, and Orion Holtey to conduct a partial inspection of the Great Oak Landfill on Old Cedar Falls Road in Randleman, Randolph County. 1. The Great Oak Landfill is a municipal solid waste (MSW) landfill owned by Randolph County and operated by Waste Management of Carolinas, Inc. 2. The facility was issued a permit to operate Phase 1, Cells 1A and 1B in conjunction with a permit to construct Phase 1 on April 10, 2017. 3. The facility currently operates in Cells 1A and 1B. 4. The partial inspection was conducted to observe Phase 1, Cells 1A and 1B secondary liner leachate line cleanout flushing and camera inspection. 5. On September 15, 2017, the SWS was informed that on September 1, 2017 during a heavy rainstorm the end cap for the secondary liner leachate line cleanout at Phase 1, Cell 1B either came off or was not in place. Thus, it is estimated that several thousand gallons of muddy stormwater entered the secondary liner leachate line cleanout pipe and secondary liner sump. It was stated that the muddy stormwater traveled along the secondary liner leachate cleanout pipe from Phase 1, Cell 1B up to the Phase 1, Cell 1A secondary liner leachate line cleanout pipe located at the landfill leachate line (primary and secondary) cleanout header, blowing the end cap off. 6. Mr. Moore stated that the secondary liner action leakage rate (ALR) was exceeded, although it has since returned to a level below the allotted maximum. Leachate sump data/ALR records were observed. 7. During the secondary liner leachate line cleanout flushing and camera inspection the lines (from the landfill leachate line cleanout header at Phase 1, Cell 1A and the secondary liner leachate line cleanout at Phase 1, Cell 1B) appeared free of blockage. Although, the secondary liner leachate line was only partially camera inspected. 8. The facility will submit a report, to include video, to the SWS detailing the flushing and camera inspection. 9. The facility will also put into place the “controls” outlined in the SWS email dated September 18, 2017. 10. The facility is expected to meet the requirements of the abovementioned email in a timely manner. Please contact me if you have any questions or concerns regarding this inspection report. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
