PURPOSE OF SITE VISIT: Routine
STATUS OF PAST NOTED VIOLATIONS:
No violations noted on the previous inspection conducted on 10/09/15.
OBSERVED VIOLATIONS:
1) Upon review of the submitted records, it was noted that the monthly application rate for Coastal Bermuda
overseeded with rye was exceeded on Field 1 for the month of December in 2013. Exceeded the monthly
application rate is a violation of Rule .0838(a)(18) that states “Approved nutrient management plans shall be
followed.” The maximum annual application rate for Field 1 was not exceeded for the same year.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) Required signage present along access road before you get to the site. Sign could use some work.
2) Access road is well maintained.
3) Some markers missing for Field 7. Getting ready to cut the field.
4) Current applications on Field 5. All septage discharges appear to be within the permitted boundaries of the fields.
5) No application issues noticed today.
6) Some weed control, mainly grasses, needed in all fields. Getting ready to cut some of the fields. Waiting on the
weather to cooperate.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) No issues noted with setback distances.
10) Application for permit renewal received on 01/08/15. Reviewed and modified the nutrient management plan with
Russ. Made changes to the nutrient management plan concerning the monthly rates, field identifications, and 80 %
crop coverage. Monthly application rates to Coastal Bermuda overseeded with rye adjusted for the higher
permitted maximum annual application rate of 100,000 gal/ac/yr. Monthly application rates for Coastal Bermuda
should match now within the nutrient management plans for SLAS-59-02 and SLAS-59-05.
11) Also, reviewed the land application logs. The monthly application rate for Field 1 was exceeded in December
2013. The nutrient management plan has a rate of 5,000 gal/ac for the month of December. Records showed that
10,882 gal/ac was applied. Since SLAS-59-02 is permitted at a higher rate, 100,000 gal/ac/yr, for Coastal
Bermuda overseeded with rye, I calculated the overage based on the same monthly application rate of 10,000
gal/ac as allowed for SLAS-59-05 in the month of December.
12) Application records for 2015 thru 2016 was emailed on 08/01/16 as requested.
