STATUS OF PAST NOTED VIOLATIONS:
An observed violation was noted to Rule .0835(g) for failure to get application for permit renewal submitted prior to the
permit expiring. Application for permit renewal was received on 12/21/15. Also, an observed violation was noted for
Rule .0838(a)(2) for not having a “No Trespassing” sign as you enter the site. “No Trespassing” sign present today.

OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) The “Septage Land Application” site and “No Trespassing” sign present at the entrance to the fields.
2) The access road is in good shape.
3) Markers present.
4) No septage discharges noticed outside of the permitted boundaries.
5) No application issues noted today.
6) Ryegrass present on the field. The ryegrass needs to be harvested.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) No change noticed that would affect setback distances.
10) Application for permit renewal received on 12/21/15.
