STATUS OF PAST NOTED VIOLATIONS:
No violations noted from the inspection conducted on 08/04/15.
OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) Site visit performed to re-evaluate areas of Fields 1 and 2a that had been buffered due to wells. The wells
to the east of Fields 1 and 2a have been officially abandoned. See pictures below. Documents have been
submitted certifying the official closure of the wells.
2) Required signage present as you enter the site.
3) The access road is well maintained.
4) Markers were up on all fields. Sammy asked about combining Fields 2a and 2b. Fields cannot be
combined because each field has a different crop. Will change the field numbers. Field 2b will become
Field 2. Field 2a will become Field 3. Field 3 will become Field 4.
5) Discharges within field boundaries.
6) No evidence of any thick applications. Evidence of that a septage application ponded some in Field 2a.
See picture below. Also, there was a grease burn within Field 2a. See picture below. Sammy was going
to have employees scrape the area and sow seeds.
7) The rye had been planted on all fields and was up. Some winter broadleaf weeds such as chickweed and
henbit were present. Weed control needed.
8) pH not checked. No applications while I was on site.
9) No objectionable odors noticed today.
10) The current setbacks to two wells is being re-evaluated due to documentation received about their official
closure. Permit currently up for renewal.
11) Records submitted with application for permit renewal. Records are under review.

Grease burn in Field 2a. Evidence that septage had ponded some where the
grease burn was seen.
