PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: Violations to Rules .0838(a) (12) and (13) was noted on the inspection conducted on 05/12/16 for septage discharged outside of the permitted boundary and markers missing from Field 1. No septage discharges noticed outside of the permitted boundaries today. Markers present for Field 1. OBSERVED VIOLATIONS: NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: 1) Required signage present. 2) Access road is well maintained. 3) Most markers present. One marker missing at the northwest corner of Field 2. Please place marker back up. 4) No septage discharges noticed outside of the permitted boundaries of both fields. 5) No spot dumps or thick applications noticed today. Ruts observed in Field 2 where it appears that there was a lot of truck traffic cutting across the field to haul some material to the west of Field 2. See picture below. Ruts need to be filled in. Please avoid truck traffic cutting across one of your septage land application field. The truck traffic destroys you crop, creates ruts, and causes unnecessary soil compaction. 6) The bermudagrass had been harvested. Will need to get the small grain planted soon. 7) No application at time of inspection. pH not checked. 8) No objectionable odors noticed. 9) Setbacks appear to be ok. 10) Records have been submitted along with the application for permit renewal received on 05/23/16. 11) I collected a composite soil sample from both fields.
