PURPOSE OF SITE VISIT: Routine
STATUS OF PAST NOTED VIOLATIONS:
No violations noted on the previous inspection conducted on 01/15/16.
OBSERVED VIOLATIONS:
1) Septage had been applied outside of Field 1 along the western edge. Septage applied outside of the
permitted boundaries is a violation of Rule .0838(a)(12) that states “All septage discharges, including aerial
drift from discharges, shall be made within the permitted boundaries of the land application site.” Any
septage trash remaining on the ground need to be cleaned up.
2) Markers were missing along the western edge of Field 1. The absence of markers is a violation of Rule
.0838(a)(10) that states “Disposal area boundaries shall be clearly marked on the ground while a site or
any portion of a site is in use.” The absence of markers along the western edge may have led to septage
being applied outside of the permitted boundary. Need to make sure that the markers are at the correct
location and that they are up and visible.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) Required signage present.
2) Access road is well maintained.
3) Some markers missing for Field 1 along the western edge. The missing markers are a violation of
Rule .0838(a)(10) as stated above. I marked off the location of the markers for Field 1 along the west
edge base on the current permit map. Marker were placed at the locations while I was there.
4) Septage had been applied outside of the permitted boundary for Field 1 along the western edge. See
pictures below. As stated above this is a violation of Rule .0838(a)(12). Any remaining septage trash
needs to be cleaned up.
5) Other than septage being applied outside of the permitted boundary for Field 1, no other application
issues noted.
6) The small grain needs to be harvested off of both fields.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed.
9) Due to the missing markers, the setbacks were not being met along the western edge of Field 1.
10) Records have been submitted along with the application for permit renewal received on 05/23/16.
