PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. This facility is permitted to receive, consolidate and transfer yard trash (as defined in 15A NCAC 13B .0101(55)), collected by the City of Winston-Salem, for storage and removal. 2. Yard trash, mainly leaves, is brought to the facility by the City of Winston-Salem Sanitation Division collection trucks. The material is then loaded into transfer trailers and transported to the city’s compost facility, Forum 52, for processing and composting. 3. The facility operates only during leaf season - typically from October through February. 4. The entire facility is paved. 5. A chain link fence surrounds the facility, and is secured with two locking gates – one for ingress, one for egress - when the facility is not operating. 6. Proper signage was observed at the facility entrance gate. 7. A 50’ buffer is required between waste and the property lines. To ensure that the buffer is maintained during operations, a dashed white line is painted on the asphalt marking the 50’limit of the buffer. 8. No yard trash, processed or unprocessed, was stored onsite at the time of this inspection. 9. A portable ramp is staged at the site for use in loading trucks and trailers with leaves prior to transport. 10. Mr. Fulmer explained that the ramp is usually placed near the center of the site. Trucks enter at the eastern gate, dump their loads then exit through the western gate. A loader is used to move and consolidate the waste before loading it onto transfer trailers. 11. There is no building onsite, so the Permit to Operate and the approved operations plan are made available for staff at the site by the operation supervisor. Mr. Fulmer stated that he maintains these documents at his office, along with waste volume records used to compile the annual facility report. 12. The site is sloped to avoid ponding and to direct water to the city’s storm drains. However, the asphalt paving had eroded in three small areas, and some water was observed to be ponding in the depressions created by the voids. Please ensure that maintenance is performed on the eroded areas in the asphalt paving to level the site and eliminate the problem of water ponding in and around waste that will be stored and consolidated at the site during periods of operation. 13. Tonnage records, as included in the annual facility report were reviewed. Records of waste volume are converted to tonnage based on a formula of weight per cubic yard. An estimated 10,245.01 tons of waste were received at the site during 2016-17 seasonal operations.
