No violations noted from the previous inspection conducted on 01/15/16.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Required signage present before you get to the site.
2) Access road ok.
3) Markers present and visible.
4) No discharges noticed outside of the permitted boundaries of the field.
5) Site seems to be getting a lot of use. No spot dumps or thick applications noticed.
6) Bermudagrass present however, weed control needed.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed.
9) Setbacks need to be checked. Not sure all markers are correct.
10) Debris being placed around field. Debris cannot be in the field or in the buffer area. Need to make sure
that you have a permit to place debris on site. See pictures below.
