FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected new vehicle -tag#HR5507, 950 gallons, VIN3C7WRMBL3HG656041. The tank appears structurally sound and securely attached. Outlet covers were on and no drips/leaks were noted. 2. Inspected the lettering which was adequate except that the town name was less than three inches on both sides of the vehicle - but it has been corrected and photos were provided by this firm to document the corrected lettering. 3. Requested a page of the truck logs which I did receive by email – upon review it appears that the required information for the pumping log is being maintained at this time. 4. Did not inspect the other permitted vehicles at this time. Please contact me if you have any questions or concerns regarding this inspection report. _ Phone: (910) 433-3352 Connie S.
